PHA Yearly Plaque Order Form

I have had requests for info required to order plaques. Here is the required info:

Your request needs to contain the DRIVER'S NAME and the CLASS.
The 7"X9" Plaque, space for 4 event plates is $15.37.
The 10 1/2" X 13" Plaque, space for 8 event plates + year end plate is $29.68

Send your name and class with check or money order to:

If you order your plaque early enough, it can be picked up and delivered at our November 12 Banquet.
I will send the request form to Dale to be placed on our web site.
